I understand and agree that personal information, collected from me, my parent or guardian, such as my name, Unique Student Identifier, date of birth, contact details, training outcomes and performance, or sensitive personal information (including my ethnicity or health information)  (together Personal Information) may be collected, stored and used by Wesley Vocational Institute or Wesley Mission (on behalf of the Wesley Vocational Institute) (Wesley).

How Personal Information is collected

I understand that Wesley will collect Personal Information directly from me, where possible, such as through online forms, email, or phone. In some cases, Wesley may also collect information from third parties (for example, your employer, training provider, or another government agency), but only where it is lawful and necessary to do so.

Use of Third-Party Student Management System

I acknowledge that my Personal Information is stored and managed in a third-party system, currently operated by ReadyTech, which provides secure data hosting and management services on Wesley’s behalf. The system provider may hold this information on servers located in Australia.

I understand that Wesley will take reasonable steps to ensure that the third-party provider:

·       complies with the Australian Privacy Principles (APPs) or equivalent data protection standards, and

·       implements appropriate security and confidentiality measures to protect my Personal Information from misuse, interference, or unauthorised access.

Disclosure of Personal Information

I authorise Wesley to disclose my Personal Information:

·       to a third-party system provider for the purpose of hosting or managing its records,

·       to other parties as required or authorised by law (e.g., regulators, courts), and

·       with my consent, to other relevant organisations or individuals.

Personal Information may be disclosed to the Department of Education (the Department).

The Department may disclose my Personal Information to other Australian government agencies, including those located in States and Territories outside New South Wales.

The above government agencies may use my Personal Information for any purpose relating to the exercise of their government functions, including but not limited to the evaluation and assessment of my training, the determination of my eligibility to receive subsidised training or for any Fee Exemptions or Concessions.

I consent to the collection, use, storage and disclosure of my Personal Information in the manner outlined above.

I also acknowledge and agree that the Department may contact me by telephone, email or post during or after I have ceased training with Wesley Vocational Institute for the purposes of evaluating and assessing my subsidised training.

General

I acknowledge that:

·      I have the right to access and correct my Personal Information held by Wesley by submitting a ‘Student Record Request WVI’ form which can be obtained via email (training@wesleymission.org.au) or phone (1800 676 039).
